ABOUT THE JOB

Vynca is seeking a dedicated Registered Nurse (RN) to play a pivotal role in our Clinical Care Delivery Team! As a RN with Vynca, you are a key part of the interdisciplinary team, helping to improve care through evidence-based guidelines for seriously ill patients through a hybrid model providing in-person visits and telemedicine visits. We’re seeking someone with a genuine passion for this important work and a deep commitment to providing exceptional patient care. This is a hybrid role that requires traveling throughout northern New Jersey, combining in-person patient visits with telehealth care delivered across NJ and other states where Vynca operates. Candidates wishing to be considered must reside within 25-miles of the assigned territory due to frequency of travel.

WHAT YOU’LL DO

Patient Care

- Serve as the primary coordinator for your patient’s care needs, facilitating services across the interdisciplinary care team to ensure all patient needs are being met

- Manage patients assigned to your Care Team, ensuring the RN and Interdisciplinary team visit frequencies align with the patient’s acuity level and clinical needs.

- Communicate with patients via phone and video conferencing as well as provide in-person visits as needed and as outlined in the care plan.
