The Virtual Primary Care Nurse Practitioner works in collaboration with a dedicated care team to provide evidence-based, primary care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients 18 years of age and older. This full-time clinical position is 100% remote. The care team will work with NP daily to drive better outcomes for the entire panel. Ideal candidate is FNP licensed in one or more states in the Western U.S. What you'll do: Patient-Centered Quality and Safety Demonstrate knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional care patients Perform history and physical assessments of patients that are accurate, complete, concise, relevant and appropriate for age, gender and clinical issue Evaluate and treat patients' medical needs with safe and high-quality care Counsel and educate patients and families Document all patient care within an electronic health record in line with current policies and procedures Work as a member of the care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is provided for all patients and at all time Provide patient centered care that is compassionate, appropriate and effective for the promotion of health, prevention of illness and treatment of disease Referral to appropriate Specialists and patient care components as appropr...
