Join our team as a Remote Case Manager RN and play a critical role in guiding members toward better health outcomes through personalized care coordination. What You’ll Do Conduct comprehensive clinical assessments and develop individualized care plans Coordinate inpatient reviews and discharge planning Advocate for members and connect them with appropriate services and resources Monitor care plans and adjust strategies to ensure optimal, cost-effective outcomes Use motivational interviewing and coaching to support behavior change and engagement Identify quality of care issues and contribute to program improvements Deliver exceptional member service in a remote environment What You Bring - REQUIRED Active, unrestricted RN license (with ability to obtain multi-state licensure paid for by the company) 3+ years of acute care RN experience Strong clinical judgment and decision-making skills Excellent communication, organization, and multitasking abilities Proficiency in Microsoft Office tools Preferred Qualifications Compact RN license Certified Case Manager (CCM) Case management or managed care experience Telephonic care or customer service experience Oncology care experience Schedule Monday–Friday, 8:00 AM – 4:30 PM (local time) Occasional evening rotation (10:30 AM – 7:00 PM) Rotational evenings, weekends, and holidays based on department needs Education Associate ...
